Wikipedia Describes The Offside Rule, As :
A player is in an offside position if he is in his opponents' half of the field and is nearer to his opponents' goal line than the ball, and all but one (or all) of his opponents. A player level with the second to last opponent is not in an offside position.
So, This Means, A Player Is Offside If He/She Is Past The Last Defender.
'nearer to his opponent's goal line' meant that "any part of his head, body or feet is nearer to his opponents' goal line than both the ball and the second last opponent. The arms are not included in this definition."
In Summary, A Player In An Offside Position Includes Head, And Parts Of His Body Except His Hands , And Is Infront Of The LAST DEFENDER.

Okay, The Offside Trap Is Really Simple .
Basically, The Defenders, And The Keeper Leave The Posts, And Go Upfront.
And When The Opposition Comes From Anywhere, They'd Be Offside .
But, They Can Beat This Easily.

the offside trap is a very effective modern defensive tactic for groups and for the team.
there are three basic countermeasures against the offside trap:
- high square passes behind the forward-moving defense line.
since the ball is in the air for a relatively long time, out of reach of the opposition, midfielders have a good chance to take possession of it.
- break through dribbling from the rear. a midfielder, moved into the field by a back pass, attempts to break through with fast, explosive dribbling.
- wall and double passses in a tight space. a wall pass, particularly in the variant of a delayed kick, to center forward has the best chance for success.
